Unless you got serious hygiene problems, or your balls have been mad sweaty for a bit, your dick will have far less bacteria on it than your hands.
I have hand eczema that flares up with harsh soaps. I use hand sanitizer instead, it’s small and fits in my pocket. Also I think that some sinks are dirtier than my ass.
Can confirm, I shit in the sinks because they are cleaner than the bathroom and using the faucet as a bidet is gentler than the communist-era 0.25ply (sand)paper.

As a truck driver in the US, depends on the restroom.
More than a few of the restrooms I’ve visited over the years had no soap or nonfunctional faucets, usually at the smaller truck stops. I started keeping handwashing supplies (separate from my daily hygiene bag) in the driver side cargo hatch of my truck. Access to potable water being often times limited, it would often be hand sanitizer, if I could get it, or windshield washer fluid, sometimes both. Not a great solution, but you work with what you have.
As for the folks that just don’t wash, ya, I’ve no clue.
Just FYI: If you’re not drinking it, it doesn’t need to be potable.

I meet the guy in the public bathroom stalls regularly, and he does NOT wash his hands, EVER.

Those that don’t wash their hands touch the door handle leaving the restroom. They also likely touch the main door handle leaving the store. If you wash your hands, aren’t you undoing it immediately?
I thought about this a lot at my last office I worked at. We had a wide variety of washers, non-washers, religious who also washed their feet (over and under socks), and people who washed and used the paper towel to open the bathroom door but subsequently opened the next door bare handed, and the germophobe who never touched any surface barehanded.
There’s no good answer. Though, metal is a pretty bad surface for germs to live on if that helps. My recommendation is to just wash your hands and then try not to think about the rest.

Worked with a lot of people from India. I’m guessing it’s from one of the many Hindu sub-religions (is that the proper term?). They worship a lot of different gods in different ways. Though some were Muslim and did call to prayer in the afternoons. Noticed some would slide the shoe and sock off and wipe down the bottoms of their feet after washing their hands. Others didn’t go quite that far and would more touch the bottoms of their socked heels with a damp hand. I presume a more symbolic gesture than anything. Which is why I also assume it’s religious and not for hygiene.
It was only a few from hundreds that I saw this behavior from.
Wasn’t super crazy as the washing of hands and feet is also symbolic in Christianity as well.

I have a small plastic freezer bag in my pocket, which I use turned inside-out to open the door.
My GF has severe (no, severe) OCD, and among all the rituals that are, objectively, pointless… This is the one that I wholeheartedly understand and agree with.

This is the real answer. Don’t even think about anything else, just wash your hands every time visiting the restroom and touching anything in there that others you don’t know have touched.
Also wash hands every time coming from outside. Just normal sanitary behavior to prevent illness. Hands get very dirty very easily outside. More than we think.

Sorry, it was a joke response, I thought I was on Shitty Ask Lemmy. Obviously I wash both before and after… though not because I touched my wang. (If yours isn’t cleaner than your hands, you’re doing something wrong.) It’s because I touched other things in the bathroom and people aren’t great about washing their hands.

It’s not cleaner than my hands while I’m peeing. That’s the reason why you need to wash your hands when touching your dick while going to the bathroom.
I try not to touch anything in a public bathroom. Including the handles on the sink. I’ll use one of those automatic alcohol sanitizer globes everyone has since COVID so I never have to touch anything other people have had their hands on.
If there isn’t paper towel I don’t bother. No point in washing my hand then touching the dirty door handle.

When I was little I’d run away because the sound of flushing scared me.
I wash my hands before using a public washroom because I know where my penis had been. It’s freshly showered and has been in freshly laundered underwear. I don’t know where the other hands that have touched the surfaces in the washroom have been. If I’m just peeing my hands are cleaner after I wash my hands then pee than they were when I entered the bathroom. If I’m deficating I wash my hands before and after.
I avoid this by not peeing on my hand.
